Child Coaching Agreement

1:1 Coaching Agreement for Under 16’s – to be read and signed by parent or legal guardian

1. I understand and agree that I am fully responsible for my child’s physical, mental and emotional well-being.  I am aware that I can choose to discontinue coaching sessions at any time.

2. I understand that “coaching” is a professional relationship that is designed to facilitate the creation/development of my child’s emotional understanding and self-regulation.

3. I understand that emotion coaching is a comprehensive process that may involve looking at all areas of my child’s life, including home/family, health, spiritual, relationships, education and recreation.

4. I understand that emotion coaching does not involve the diagnosis or treatment of mental health disorders as defined by the British Psychological Society and that emotion coaching is not a substitute for counselling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment and I will not use it in place of any form of diagnosis, treatment or therapy for my child.

5. I confirm that if my child is currently in counselling, therapy or under the care of a mental health professional, that I have consulted with them regarding the advisability of working with a coach and that this person is aware of my decision to proceed with the coaching relationship. Please note, it is in the best interest of your child to inform their coach of other professional involvement.

6. I understand that information will be held as confidential unless agreed otherwise, in writing, except as required by law.

7. I understand that certain information may be anonymously shared with other coaching professionals, eg. for supervision, for training OR consultation purposes.

8. I understand that it is my responsibility, as the client’s parent/guardian, to inform MITOS in writing in the event that I wish to end my coaching relationship. At this point MITOS will remove my personal contact information from their records and the onus will be on me to resume contact should the need arise.

9. I understand I am to inform my child’s coach if, for any reason, my child cannot attend sessions. I understand that should sufficient notice not be given, I will still be charged.

10. I understand that I can contact my child's coach for a 10min scheduled call and I will not incur a charge. Any call lasting more than 10mins will be charged at £35, and then a further £35 per 30mins thereafter. You will be informed of when this occurs by your child's coach.

11. I am aware of the MITOS Safeguarding and Privacy Policies, including data protection.
